- Letter to the director of IBESR in Haiti (Haitian Social Services). Specify in the letter the age and gender of the child you are interested in adopting. Briefly include your family make-up and how you intend to raise your adopted child.
- Original birth certificate for each parent.
- Original marriage certificate.
- Divorce decree, if any.
- Four I.D. photos (passport type) for each parent.
- Original Homestudy (notarized by social worker).
- Health Certificate for each parent.
- HIV, RPR, CBC and chest x-ray.
- Psychological Evaluation for each parent.
- Criminal Records check for each parent.
- Three letters of reference - in "letter" format (one must be from the pastor of the church where the prospective parent(s) attend regularly).
- Financial Statement from Bank.
- Recent T4 slips, W-2.s or 1099.
- Letter from current work place on a letter that includes the employer's full address, telephone number and e-mail address. If self employed, a letter on your company's letterhead with the address, phone number and e-mail address for your company declaring that you are self employed and stating your income.
- Statement of assets and liabilities.
- Power of Attorney made out to:
